Spark Rehabilitation Program

This clinical excellence program is tailored to individuals who suffer with cardiac and pulmonary diseases with diagnoses such as CHF, COPD, and Sepsis.        

Our Spark Rehabilitation Program’s goal is to help patients achieve their prior level of care as quickly and seamlessly as possible, while reducing the risk of rehospitalization. Our highly trained professionals assist patients in navigating their health even after discharge. Our zone tools program teaches our patients how to recognize warning signs and symptoms to assist them in managing their care at home. Leading up to discharge, we will coordinate your continuing care with your primary physician.     

We want our care to be a long-term solution for our residents to help them succeed. Individualized care plans in are created and managed in coordination with therapy, nursing and Attending Physicians for our patients to help them succeed as quickly and safely as possible.
